Actually when the US first invaded Iraq most of the deaths were caused by helicopter collisions, Vehicular accidents, and friendly fire, as the case of the professional football player from the Phoenix Cardinals that was killed by fire from his comrades..And these were not the weekend warriers, these were the pros, the full time Marines.
