I understand the general rule on treatment of intersex is to be determined by the following characteristics :

1 - Urine: If the person urinates from the male organ only, he is considered as a male; if from the female organ, then female.

2 - When the person reaches the age of puberty if the sperm comes out from the male organ several times he is considered a male. But if the person has several menstruation periods, she is considered a female.

3 - Sexual desire or attraction: This condition is considered only if the above characteristics are absent to resolve this matter. If the person feels sexual desire towards the female, he is a male and the reverse is true. If the person feels desire for both sexes, that person is considered an epicene person (having characteristics of both sexes).

4 - Beard-growing, breast swelling or the coming out of milk from the breast are all signs that could help define the person's sex.

And it is needed to make chromosomal analysis.  If chromosome structure is 46XY then it's male. if 46XX then it's female.  Once the sex is distinguished after chromosome analysis, necessary treatments (medication and/or operation) could be administered.  However. a committee consisting of medical experts can make the decision.

My question is:

If an intersex, who is raised as a boy and identify himself as male, is diagnosed having the characteristics of female and is genetically 46XX and the medical committee makes the decision that surgery should be done to correct him/her gender from boy to girl,  can he refuses to have treatment or have the treatment otherwise to remain as a boy?

This is definitely something you have to look inside to find the answer.  No one can tell another person what sex he/she SHOULD be. Making the matter public will only create animosity towards one another. Maybe that person was built that way for a reason. This is not a disease that should be "fixed." Being happy with who you were made to be seems the best answer here, but only in my opinion.
